  • Have a personal or family history of medullary thyroid carcinoma (MTC)

  • Have been diagnosed with multiple endocrine neoplasia syndrome type 2 (MEN 2)

  • Have ever had thyroid cancer

📢 Animal Studies: In studies with rodents, GLP-1 medications caused thyroid tumors, including thyroid cancer. It is not yet known whether they cause the same effect in humans.

Kidney Problems/Kidney Failure – Vomiting, nausea, and diarrhea may cause dehydration, worsening kidney function in those with pre-existing kidney conditions. In severe cases, this may require hemodialysis.

Thyroid C-Cell Tumor Risk – Trouble swallowing, hoarseness, a lump or swelling in your neck, or shortness of breath. (See Black Box Warning below.)

Pancreatitis – Severe abdominal or back pain that will not go away.

Acute Gallbladder Disease – Symptoms include:

  • Pain in the middle or upper right stomach

  • Fever

  • Yellowing of the skin or eyes (jaundice)

  • Nausea or vomiting

If gallstones or gallbladder inflammation is suspected, further evaluation is needed.

Hepatitis, Elevated Liver Enzymes, or Jaundice – May indicate liver dysfunction.

Diabetic Retinopathy – Worsening vision problems in individuals with Type 2 diabetes, particularly those with a history of diabetic retinopathy.

Severe Gastrointestinal Disease – This medication may worsen gastrointestinal conditions; do not use if you have a history of severe gastrointestinal disease.

Semaglutide may cause low blood sugar (hypoglycemia), especially when taken with insulin or insulin secretagogues such as sulfonylureas (glipizide, glimepiride, glyburide).

* Signs of Low Blood Sugar:

  • Dizziness, weakness, shakiness, jitteriness

  • Headache, confusion, slurred speech

  • Anxiety, mood changes, rapid heartbeat

  • Sweating or lightheadedness

💡 What to Do:
If you experience low blood sugar symptoms, seek in-person care immediately and try consuming a sugar-containing food or drink (e.g., juice, regular soda, or hard candies containing 15–20 grams of sugar).

*If you are taking insulin or sulfonylureas, speak with your healthcare provider about adjusting your dose to lower the risk of hypoglycemia.
